This week’s Monday Memories focuses on the 12 Documents You Need to have that will help and/or your loved ones during health, life and even during death.

During this video, I discuss the Triangle.  Each part of the triangle is important to your success.  The three parts of the triangle are Administration,  Money, and Health.  Below the video, I list the link to RocketLawyer a trusted service that I use for most of the forms discussed in this video.

Here are the documents you need.

Administration:

Power of Attorney

Marriage Certificate (if applicable)

Divorce Decree (If applicable)

Finance:

Life Insurance Policies/ Trust Documents

Tax Returns (at least 5 years)

401k, Stock/Bond Certificates,

Real Estate Deeds, Cemetery plots, Mortgage coupons

Proof of Cancelled Debt or Outstanding loan (in case someone owes you)

Vehicle Title

List of all banking accounts including safe deposit boxes.

Health

Living Will (Should include Do Not Resuscitate order if applicable)

Healthcare Power of Attorney

For many of the documents, I recommend that you use the “State-Specific” documents created by your attorney or a service life RocketLawyer.
